A. All information shall conform with the Rabies Vaccination Certificate developed by the National Association of State Public Health Veterinarians, Inc.

B. Disposition of Records. Upon performing the immunization of an animal, the veterinarian shall complete the prescribed form and give the owner the original copy thereof. The second copy shall be forwarded to the Animal and Rabies Control Center and retained for not less than two years for reference in the event verification of immunization may be desired at a later date.
